For individuals who's company offered the 6-week employee wellness challenge.  Individuals who have completed the challenge can get 8-weeks of ongoing support.  

You completed the 12th State Nutrition 6-week challenge with success!  Are you ready to continue with mindfulness and accountability to keep the success going?

You're in luck!  We want to support your success with ongoing wellness support. 

12th State Nutrition will lead ongoing support beyond the challenge.

What it includes:
  • Ongoing access to the personalized app
  • Continued Holistic focus
  • Individualized habits in the app related to nutrition, movement, sleep and stress reduction (specific to you) (in addition to the group habits)
  • Monthly 1:1 meeting
  • Twice weekly check ins via the app
How it works:
  • Ongoing support is hosted in 8 week blocks with the option to continue with the program at the end of each two month cycle.
  • At the end of each 8 week cycle you can decide to continue with the program or pause participation.  
  • If your company offers a reimbursement details for that will be in your company email invite.
